Amy Ford

She joined Teleios with 14 years of clinical expertise and 12 years of leadership experience.  Amy focuses on enhancing Palliative Care programs and optimizing Hospice operations for client organizations. 

Amy’s career began as a Hospice field nurse, where she developed a deep-rooted passion for delivering high-quality end-of-life care.  Amy is appreciative of the early career support and mentorship that facilitated her career progression into various leadership roles.  

In addition to leading Hospice operations, Amy has taken on executive leadership roles overseeing home health operations, Palliative Care (of varying models), business development, and patient access.  Her persistent efforts have resulted in innovative clinical pathways for community-based support of seriously ill individuals and opened the door for collaborative relationships with acute care health systems paving the way to integration of Palliative and Hospice Care.    

Amy's unwavering dedication to championing the needs of both employees and patients is the cornerstone of her success.  As a tireless advocate for patients, she is committed to addressing critical issues and removing barriers to care. 

In addition to her RN licensure, Amy holds an MBA in Healthcare Management.
